Nina Feiman - School Nurse - 793-6130 x 5313

There has been a steady increase in the number of children with viruses, the flu, and infections. In an effort to reduce the incidence of illness among the children and faculty, it is essential that parents/guardians adhere to the district policy: children with fever, vomiting, diarrhea, undiagnosed rash, pink eye, excessive coughing or sneezing, need to remain at home for at least one full day after the symptoms pass. Children who are sent home from the Health Office due to illness will NOT be permitted to return to class the following day.

Students who will be entering Grade 4 in September, 2008, must provide a current physical exam report. The report is due in the Health Office no later that 10/1/08 as mandated by New York State.

For your convenience, I sent home a medical form for your child’s doctor to complete and sign.

If you have any questions or concerns, you may reach me at 793-6130, ext. 5813.

Thank you for your cooperation.
