Physical Examinations

ALL 2nd and 4th graders and new entrants are required to submit results of their current physical examination to the school nurse. Please note that all physical examinations must be within the year (dated on or after September 1st, 2007)

Medication Administration

If you anticipate that your child needs to take medication, either prescription or non-prescription, during the 2008-2009 school year, you must have your doctor complete the attached form. A parent should sign at the bottom of the form.  Please note that I need a new form completed each school year.  Once this form is complete, return the form and the medication in its original container.  All incoming 2nd and 4th graders should note that the present physical form also contains a medication consent form under medications and a place for parents to sign at the bottom of the physical form.  This form also will suffice.

FROM THE NURSE'S CORNER:

Parents are often faced with the decision of whether or not to send their child to school. The school is responsible for maintaining a healthy environment for all children. Parents are encouraged to consult their own physicians when they have concerns. I am available to answer any questions that may arise. The following guidelines are presented to promote the health of the sick child and the entire school community.

SOME GUIDELINES TO FOLLOW WHEN DECIDING IF YOU SHOULD SEND YOUR CHILD TO SCHOOL

Fever-If your child has a fever, he or she should not come to school. A child should remain home 24 hours after the fever has ceased.

Cold & Cough-If your child has a bad cold and cough and /or a persistant

runny nose, he or she should remain at home until symptoms subside.

Sneezing and coughing will spread a cold very quickly. Colds are most

contagious the first 3 days.

Rash-Please consult your physician if your child has a rash. A doctor’s note is required for any rash other than eczema. For example, chicken pox, impetigo and 5th disease are rashes that need a doctors note. Specific restrictions regarding rashes differ. I can discuss this with you on an as needed basis.

Vomiting or Diarrhea-Child should remain at home if he has been up with these symptoms during the previous day or night.

Chicken Pox-Child should be kept home from the initial onset of chicken pox

until all lesions are dry.

Conjunctivitis-(pink eye)–Child should stay home if he or he is diagnosed with conjunctivitis until there is no discharge and the eye no longer appears pink.

If the child is diagnosed with allergic conjunctivitis (which is not contagious)

a doctor’s note is required.

Head Lice-If a child has head lice she or she cannot be re-admitted to school until all nits (both viable and non viable) are removed. Contact your doctor for advise on treatment and follow all directions carefully. Eastchester School District has a “no nit” policy.

Strep-If you are waiting for the results of a strep test do not send your child to school until you have negative results. The school physician advises that children with strep return to school only after they are on medication for 48 hours.

Contagious Diseases-Please notify me if your child has been diagnosed with the following diseases: chicken pox, scarlet fever, strep, ringworm, impetigo, conjunctivitis, 5th disease, coxsachie virus and lice.


Parents are asked to call the nurse’s office in the morning when they decide to keep their child at home. The child’s name and reason for absence should be stated. If there are any questions please call me at 793-6130 ext 5111.
