| The following forms are to be printed only if your child does not have an eligible physical examination on file in the Health Office. (Eligibility lists are posted in various places in the FMS/HHS complex). Please download both pages and have your child's physician complete both pages when your child is examined. Please return them to the Health Office along with the completed Athletic Packet by the due date stated on the Information Top Sheet |
