Interval Health History (30 day re-check) forms are due in the nurse's office prior to the beginning of the fall season. Anyone who received a physical by the school physician, or a private physician, more than 30 days from the onset of tryouts/participation (see below for dates) needs to submit a form to be cleared by the nurse. Without the form properly filled out, signed by a parent, and filed in the nurse's office, participation will NOT begin. The nurse will be available in her office at the folowing times:

Monday,        July 28,  5 - 7 pm

Wednesday, July 30, 5 - 7 pm

Wednesday, August 20, 7:30 am -9:30 am

Monday, August 25, 7:30 am-9:30 am

Fall Sports Start Dates:

Varsity/JV football begins on 8/20/08

All other V/JV sports begin on 8/25/08

Middle School begins on 9/3/08
