New York State law requires physical examinations by a family or school physician for students in grades 2, 4, 7, and 10, as well as for new entrants. The District recommends an examination by a family doctor. The physical forms are available in the Nurse’s office and will also be mailed home in the spring prior to the student’s entering the required grade. Registration of new students is conditioned upon providing proof of a medical examination (within the past year) and required immunizations, including Hepatitis B. Certificates of immunization are required for every student in Eastchester. Children must be immunized against polio, diphtheria, measles, mumps, and German measles (rubella). In addition, the Dtap booster is now required for all sixth grade students, age 11 or older, if the last booster shot was more than 2 years ago. The only exception to the immunization requirement is medical or religious exemption based on legal documentation.

Medication at School

If a child must receive medication at school, state law requires:

  • A note from a physician stating the child's name, date, name of medicine, dosage/time to be administered, and possible side effects
  • A school form authorizing the school nurse to administer the medicine
  • An adult must bring the medicine to school in its original container
